Case Management and Utilization Review

Brannon House, our RN Case Manager, functions within the hospital setting as a patient advocate. She communicates with the patients, physicians, and the insurance companies to ensure the best possible care is given to the patient based on national standards set by the American Medical Association.

This process begins with admission to the hospital. Patients are assessed for discharge needs and assisted with providing these needs. Discharge planning might include home health, skilled nursing placement, home medications and equipment. Utilization review provides insurance companies the information needed to provide financial coverage for a hospital stay. This department demonstrates to the insurance company the seriousness of your illness, by providing diagnostic results and physician plans of treatment to the insurance company.

 Our hospital offers swing bed services, also known as skilled nursing services for patients who have progressed from acute care to sub-acute care. In other words, these services are available for our patients who no longer need inpatient care but may need more additional assistance and therapy to ensure a safe discharge. We offer daily activities to patients including physical therapy, speech therapy, occupational therapy, mental health services, intravenous therapy, and nutritional counseling. By choosing to receive these services, our patients get to stay in their hometown, getting care closer to their friends and family during this short-term rehabilitative stay.

When and if the time comes, our Case Management department is also happy to help you find long-term care and/or hospice care should you or your loved one need it. We are participating providers with most insurance plans.
